Details of the Response
Thank you for your request under the Freedom of Information Act. We cannot release personal details under the Freedom of Information Act.
However, we can supply copy medical records under the Data Protection Act and I attach the application form and guidelines for your information.
Please note before we able to release copy records it is necessary for you to provide evidence that you are the personal representative of the deceased. This should be either a copy of the Will, a copy of the letters of Administration, or a copy of the Grant of Probate. If you do not have any of these items, then we need to have a letter from the Solicitor dealing with deceased’s affairs.
