End of Life Care
The Trust’s End of Life Care Steering Group has responded:
- Does the organisation have an End of Life Care Strategy? – If so please would you send an electronic copy?
The Trust is currently working towards a finalised End of Life Care Strategy, available in December 2010.
- Would you also send any condition-specific End of Life Care strategies that you have developed, for example, for cancer, liver disease, dementia etc?
Trust-wide, we have adopted the use of the Liverpool Care Pathway (LCP) for all patients regardless of diagnosis. There is funding in place to support the recruitment of an LCP facilitator – this post is currently being advertised. A Heart Failure Pathway is currently being developed for piloting across community and acute services. A Dementia Strategy Group has been convened to develop a pathway for this patient group. Cancer Pathways are site specific and include End of Life Care planning – these pathways have been developed in line with SWSH Cancer Network requirements and have been built into the Trust’s cancer business plans.

The End of Life Care Strategy has resulted in the creation of an End of Life Care Strategy Steering Group. This group is currently implementing self-assessment against the Quality Markers for Acute Trusts. The End of Life Care Strategy Steering Group supports the End of Life Care Working Group, responsible for actioning any recommendations.
- Who are the End of Life Care leads in the organisation?
The Trust has joint End of Life Care leads. These include the Deputy Chief Nurse (Vanessa Avlonitis), Head of Pastoral Care (Judith Allford) and Senior Nurse Specialist in Palliative Care (Karen Cook).
