Details of the Response

Thank you for your request to Ashford and St. Peter’s Hospitals NHS Trust regarding Prescribing Formularies and guidelines.

Please could you provide the following:
  • A copy of your most recent Prescribing Formulary?

    The formulary is currently being updated - due to be completed at the end of the financial year. The last copy was printed in 2001, and is too out of date to be meaningful.

     
  • VTE is now a CQUIN Target, due to this could you please provide your most recent Venous Thromboembolism (VTE) clinical policies or guidelines? 

    A copy of the Thromboembolism Risk Assessment Tool was sent to the requester along with a patient leaflet on this subject. 
If the information is not contained within the formulary please can you answer the questions below:
  1. The name of the committee which updates the above?

    Drugs & Therapeutics Committee

     
  2. Review dates and next scheduled update dates?

    The formulary is updated after each Drugs & Therapeutics Committee meeting (every 2 months). The entire formulary is being reviewed currently and once each section of it is agreed, it will be reviewed on an ongoing basis.

     
  3. Which other organisations use the above?

    They could be our service level agreement customers e.g. Surrey borders (they have own mental health formulary but use our formulary for routine drugs) and Surrey Community Trust, however only ASPH NHS Trust would use the formulary on a routine basis.
