I would like the information in the following table. You may provide this in a spreadsheet if it is easier for you, if also provided with an organisation chart that shows the roles listed in the hierarchy of the overall organisation.

A copy of relevant organisational job role profiles:
  • Senior Information Risk Owner;
  • Caldicott Guardian;
  • Privacy Officer;
  • Registration Authority Manager;
  • Data Quality;
  • Information Governance;
  • Information Security;
  • Freedom of Information Act requests
  • Subject Access requests;
  • Health and corporate records
I would like this information for each year your organisation has existed AND completed the Information Governance Toolkit.

Details of the Response

Thank you for your request to Ashford and St. Peter’s Hospitals NHS Trust regarding resources allocated to Information Governance within the Trust.

Please find attached a table detailing the allocated resources relating to each yearly submission of the Information Governance Toolkit assessment.

Also included are documents as follows:
  • Information Governance Manager job description
  • Corporate Records Manager job description
  • Health Records Manager job description
  • IT Services Manager job description
  • Data Quality Co-ordinator job description
  • Health Informatics Organisational Structure
Both SIRO and Caldicott Guardian roles have adopted the national job descriptions for these roles.


Please note that the following information has not been included under advice received from the SHA:
  • Years in Post – S.12 exemption
  • Years in NHS – S. 12 exemption and S.40 FOI
  • Age – S.12 exemption and S.40 FOI
In order for this information to be obtained we have estimated that the appropriate limit of £450 will be exceeded.

Section 40 also applies to the age range of individuals as it is felt that this could identify certain people.
