Details of the Request
- The number of serious untoward incidents (SUIs) reported in the trust during the 2009 calendar year which involved actual clinical harm to a patient or patient death;
- The number and percentage of these in which the fact that a SUI occurred and the results of the subsequent investigation a) were and b) were not reported to the patient or the patient's relatives/carers; Please note when these numbers include a 'never event'
- Of those that were reported to patients or carers (2(a)), the number and percentage of cases where the patient or their relatives were informed about the SUI within on week (7 days) of the event occurring.

Details of the Response
Thank you for your request to Ashford and St. Peter’s Hospitals NHS Trust regarding serious untoward incidents.
- The number of serious untoward incidents (SUIs) reported in the trust during the 2009 calendar year which involved actual clinical harm to a patient or patient death

- The number and percentage of these in which the fact that a SUI occurred and the results of the subsequent investigation a) were and b) were not reported to the patient or the patient's relatives/carers
The patients and/or their carers were all informed of where mistakes were made or if in fact they had been diagnosed with an infection.
- Of those that were reported to patients or carers (2(a)), the number and percentage of cases where the patient or their relatives were informed about the SUI within one week (7 days) of the event occurring.
Unfortunately, we do not record this information in such detail, but can confirm that where patients are involved in SUI we tell them and offer to share the investigation report.
The number of SUI’s is a statement of common information and is presented in the public Board papers every month.
SUI reporting is actively encouraged and seen as a mark of an organisation with a culture of active reporting and learning.
