  1. A review of staffing levels.
  2. A review of staff training.
  3. A review of the management and leadership of nursing staff
  4. An assessment of whether your management structure ensures that clinical staff and their views are fully represented at all levels of the trust.
  5. A review of record-keeping procedures.
  6. A review of the level of specialist elderly care medical input into all wards admitting elderly, acutely ill patients in significant numbers.
  7. Please send me a copy of each review that has been carried out into the above areas.
  8. Where a series of reviews/assessments has been carried out, the most recent one will suffice.

Thank you for your request to Ashford and St. Peter’s Hospitals NHS Trust regarding the following:

Please state whether or not each piece of work listed below has been carried out in light of the Francis Inquiry into Mid Staffordshire Foundation Trust in February 2010. Please denote this with a "Yes" or "No".

  1. A review of staffing levels

    Yes A review has just been completed and consultation finished on 3rd December 2010. Implementation will commence December - March 2011 (copy sent to requester). The review results in stronger nurse/midwife leadership at all levels of the organisation, provides a better career structure for nurses, will improve clinical outcomes by improving the skill/grade mix on the wards/ departments.
     
  2. A review of staff training

    Yes

    The Trust has achieved over 90% of staff receiving an appraisal and is planning to achieve 100%. Part of the appraisal process is to ensure a professional development plan has been undertaken which incorporates the training a member of staff needs to undertake in order to fulfil the role. The Director of Workforce and Organisational Development is instrumental in developing leadership training for all staff as well as Mandatory training. This is monitored at Divisional levels and the Ashford and St Peter’s Hospitals NHS Foundation Trust Board.

  4. An assessment of whether your management structure ensures that clinical staff and their views are fully represented at all levels of the trust

    The Ashford and St Peter’s Hospitals NHS Foundation Trust has implemented a Divisional structure which was led by the Deputy Chief Executive. The implementation of the 7 Divisions will be complete by March 2011.

    Each Division is led by a Divisional Director who is a doctor. A Divisional Head of Nursing/Midwifery will report to the Divisional Director with professional accountability to the Chief Nurse. The Divisional Directors are professionally accountable to the Medical Director. Matrons are accountable to the Heads of Nursing/Midwifery and are responsible for overseeing the ward/departments with ward/department sisters being responsible for the 24 hour service of their clinical area.

    This structure enables clinical staff to raise views at every level of the organisation.

    The Chief Executive provides team briefing to staff after each Foundation Trust Board which is another opportunity for staff to raise issues, and also holds quarterly staff briefing sessions open to all members of staff.

    The Executive team undertake weekly 'walkabouts' to clinical areas to gain views. The Chief Nurse and Medical Director are very visible throughout the Trust. Non Executive Directors also visit areas of the Ashford and St Peter’s Hospitals NHS Foundation Trust to talk to staff and patients.
     
  5. A review of record-keeping procedures

    Yes

    There are various audits that take place on a regular basis which will look at record keeping. We can confirm that a clinical audit of record keeping standards is undertaken annually across clinical specialties with results discussed within clinical areas and action plans developed.
     
  6. A review of the level of specialist elderly care medical input into all wards admitting elderly, acutely ill patients in significant numbers

    The majority of patients admitted to the FT are 65 years or older. We employ eight Consultants who specialise in care of the Elderly Medicine.

    Nurses as all staff comply with the Foundation Trust's Pledge, which can be found on our website: http://www.ashfordstpeters.nhs.uk/.

    They also follow the Nursing & Midwifery Council's Code of Professional Practice which can be found on the Nursing & Midwifery Council's website www.nmc-uk.org/

    The Trust was given authorisation on December 1st 2010 to become a Foundation Trust by Monitor, the economic Regulator for the NHS. This was awarded following a rigorous process of assessment on the organisation's financial and quality governance assurance frameworks. The Trust website includes useful information and in particular specific information can be found in the Trust's Board papers which are posted monthly on the various issues that relate to this FOI request.
