To continue providing high quality health services in the face of current financial challenges, we know we need to make significant and transformational changes over the next few years.
As such we strengthened our approach to service improvement in 2014/15 and made progress with several projects. These include:
Be the Change
An initiative led by junior doctors in February 2014, which saw 88 change projects identified to take forward.
A project to reduce the number of patients readmitted to hospital within 30 days of discharge.
Patient facing pharmacy
A new way of working, with pharmacy staff spending more time on the wards and dispensing medication for discharge from ward-based hubs. This reduces the turn-around time for prescriptions and also reduces prescribing errors.
