Introducing Digital Consent
As part of our programme to make digital count for you, the Trust is introducing a new digital consent system.
You may have experienced our staff asking you to give consent for your procedures and treatments, whereby you sign a paper document.
The trust will use a new digital system to make the process simpler, and more convenient for you. Instead of signing a paper document, you will be able to review information relating to your treatment and consent using an electronic platform, either in clinic or at home. Our new secure consent platform is provided by Concentric and you can visit their website here: https://concentric.health/patients/
What this means for you
- You will receive information about your procedure in clear, accessible language, to help you make an informed decision.
- You can review your consent form at a time that suits you, either on your own device or on a Trust-provided tablet during your appointment.
- The consent documentation is presented in English, interpreters and in-browser translation tools are available to support you.
What you need to do
The Trust will phase the roll-out of digital consent. If your clinical care team is using digital consent then they will explain the process during your consultation and will send you a link to your digital consent document. You will be able to review the information, ask questions, and give your consent electronically.
If you prefer, you can still provide consent using paper or complete the digital form while you’re at the hospital.
