The majority of patients die outside of their local acute hospital in community settings such as care homes, nursing homes or their own home.
GP Community roll out was implemented last June 2022 and we have now successfully onboarded all 38 North West Surrey GP practices along with East Elmbridge PCN. GPs currently refer to the MEO via the method of Choose & Book / eRS (electronic referral service) and we aim to provide a service that supports GPs and relatives through the processes of registering a death.
Community Hospitals / Non-Acute Hospitals
We are also now working with CSH Surrey - Woking and Walton Community Hospitals as well as Runnymede Hospital (Circle Health Group) and Nuffield Health Woking Hospital.
